Forms and Requests

Many forms and requests are now available through your secure myLAGERS account.

Log in to myLAGERS to complete your request online. The following actions can be completed on myLAGERS:

  • Apply for retirement
  • Apply for disability benefits
  • Change your beneficiary
  • Change your address
  • Setup or change direct deposit
  • Apply for a refund of contributions or lump sum (eligible deferred members only)
  • Update tax withholding
  • Print 1099-Rs
  • Estimate purchase of service
  • Print income verification letters
  • Download your member annual statement
  • Run a benefit estimate

If you are unable to complete the process through myLAGERS or need a paper form, please contact LAGERS at 1-800-447-4334. We will be happy to assist you. Survivors and beneficiaries who do not have access to myLAGERS should contact the LAGERS office with questions.
